Good Offices Confidentiality and Accountability Statement

Ministers nominated to serve as Good Officers for their Chapter are asked to read the Expectations and Practices for Chapter Good Officers and complete the Chapter Good Officer Ratification Form below. Please read through the Expectations, Confidentiality and Accountability sections carefully and affirm your agreement with each. When we receive your Ratification Form, we will contact you if we have questions or need clarification. You will receive a welcome letter with information about training events and resources for Good Officers within a few weeks of completing the form. If you are unable to complete the form in one session, click on “Save and Continue” at the very bottom of the form to the right of the Terms of Service and you will be directed to a page with a link to access the form to finish later.
